  I agree to pay the yearly membership fee of £25 per month for 12 months (Total Payable £300), payable by standing order on the 1st of each month.  (Details of the standing order account details and player reference number will be provided on completion of this form and relevant ID documents being handed in) 

I agree to Springfield's FC accessing the above named players records on the FA's whole game system for the purposes of registration and team management.
